1. Acceptance of These Terms

By accessing or using the PetESALetter.com website or engaging our clinical and psychological assessment services, including Emotional Support Animal (ESA) evaluations and letters, Psychiatric Service Dog (PSD) assessments, housing accommodation assessments, behavioral health assessments, ADHD assessments, anxiety assessments, mental health assessments, and emotional support services (collectively, the “Services”), you agree to be bound by these Terms and Conditions (“Terms”).

If you do not agree, you must not use this website or the Services.


2. About PetESALetter.com

PetESALetter.com is a clinical and psychological assessment service operated by VeriMedix LLC. We connect patients with licensed mental health professionals for telehealth evaluations and clinical documentation.

Note: VeriMedix LLC also operates VeriMedix.com, which provides medical billing and revenue cycle management services to healthcare providers. These Terms apply only to PetESALetter.com and our clinical assessment services for patients. For medical billing services, please refer to www.VeriMedix.com.


3. Services Provided

PetESALetter.com provides the following clinical and psychological assessment services directly to patients:

  • ESA (Emotional Support Animal) Letters – Clinical evaluations and documentation for housing accommodations

  • PSD (Psychiatric Service Dog) Assessments – Evaluations for psychiatric service dog documentation

  • Housing Accommodation Assessments – Clinical assessments for disability-related housing accommodations

  • Behavioral Health Assessments – Evaluations for behavioral and mental health conditions

  • ADHD Assessments – Diagnostic evaluations for Attention-Deficit/Hyperactivity Disorder

  • Anxiety Assessments – Clinical evaluations for anxiety disorders

  • Mental Health Assessments – Comprehensive mental health evaluations

  • Emotional Support Services – Follow-up consultations and ongoing support


4. Nature of Services (Non-Emergency Care)

Our Services are intended for non-emergency clinical assessment and documentation only. They do not replace in-person emergency care, crisis intervention, or ongoing psychotherapy.

If you are in crisis or experiencing a medical or psychiatric emergency, call 911 (in the United States) or your local emergency number immediately, or go to the nearest emergency room.

5. Eligibility and Location

To use our Services, you must:

  • Be at least 18 years old (or the age of majority in your jurisdiction)

  • Be located in a jurisdiction where our clinicians are licensed and permitted to practice telehealth at the time of your scheduled session

  • Have access to a compatible device (computer, tablet, smartphone) with internet connection, camera, and microphone for telehealth sessions

We reserve the right to decline or discontinue Services where local law, licensing requirements, or professional standards would be violated.


6. No Guarantee of Outcome

Payment for an assessment covers the licensed clinician’s professional time, clinical evaluation, judgment, and preparation of documentation. Payment does NOT guarantee a specific outcome, diagnosis, or letter.

Our licensed clinicians may determine, based on their independent clinical judgment, that you do not meet diagnostic criteria or clinical standards for:

  • An ESA or PSD letter

  • A particular diagnosis (ADHD, anxiety disorder, etc.)

  • Housing accommodation documentation

  • Other requested clinical documentation

Clinical decisions are made in accordance with professional standards, ethics codes, and applicable law.


7. Clinical Independence

All clinical decisions, diagnoses, recommendations, and documentation are made exclusively by the licensed mental health professional providing your assessment.

Neither VeriMedix LLC, PetESALetter.com, nor any administrative or support staff may override, influence, or dictate clinical judgments or outcomes.

Clinicians operate in accordance with their professional training, licensing standards, ethics codes, and applicable federal and state law.


8. Your Responsibilities

You agree to:

  • Provide accurate information: Give truthful, complete, and accurate information about your identity, contact details, medical history, mental health history, current symptoms, medications, and any other information requested during the assessment.

  • Attend scheduled appointments: Be present and prepared for your scheduled telehealth session with appropriate technology and a private, quiet environment.

  • Use documentation appropriately: Use any ESA letters, PSD documentation, or clinical reports only as permitted by applicable law (e.g., Fair Housing Act for ESA housing accommodation requests).

  • Do not misuse documentation: Do not alter, falsify, reproduce for fraudulent purposes, or otherwise misuse any clinical documentation or letters issued by our clinicians.

Misuse of clinical documentation may violate federal or state law and may result in termination of Services and referral to legal authorities.


9. ESA and PSD Documentation

9.1 Purpose and Legal Basis

ESA letters and PSD assessments provided through PetESALetter.com are prepared by licensed mental health professionals in accordance with applicable disability and housing laws, including:

  • Fair Housing Act (FHA) – U.S. federal law prohibiting housing discrimination based on disability

  • Americans with Disabilities Act (ADA) – For service animals (note: ESAs are NOT service animals under ADA)

  • Applicable state and local laws

9.2 What ESA Letters Are For

ESA letters are clinical documents that:

  • Verify that you have a mental health condition that substantially limits one or more major life activities

  • Confirm that an Emotional Support Animal provides therapeutic benefit and is part of your treatment plan

  • Support requests for reasonable housing accommodations under the Fair Housing Act

ESA letters do NOT:

  • Grant public access rights (ESAs are not service animals and do not have public access rights)

  • Guarantee acceptance by any landlord, property manager, or housing authority

  • Exempt you from pet deposits or fees in all cases (laws vary)

  • Allow animals in “no pets” policies in all circumstances (requests must be reasonable)

9.3 No Guarantee of Third-Party Acceptance

We do NOT guarantee that any landlord, property manager, housing authority, airline, school, employer, or other third party will accept your ESA letter, PSD documentation, or housing accommodation assessment.

Each third party may apply its own policies, verification procedures, and legal interpretations, which are beyond our control.

Our letters and assessments are prepared by licensed professionals in accordance with applicable laws, but we cannot control how they are reviewed, verified, or applied by third parties.

Third-party rejection is not, by itself, grounds for a refund (see our Refund and Cancellation Policy for details).


10. Telehealth and Technology Requirements

Services are delivered via telehealth, including live video consultations, secure audio calls, and electronic communication (email, patient portal, secure messaging).

10.1 Your Technology Responsibilities

You are responsible for:

  • Having a compatible device (computer, tablet, or smartphone with camera and microphone)

  • Reliable internet or cellular data connection

  • A private, quiet, and confidential environment for your telehealth session

  • Testing your technology in advance of your scheduled appointment

10.2 Technical Issues

Technical difficulties on your side may be treated as late cancellations or no-shows under our Refund and Cancellation Policy if the session cannot be completed and you did not notify us in advance.

We recommend testing your video/audio connection and internet speed before your scheduled appointment.


11. Fees, Payments, and Refunds

Fees for Services, accepted payment methods, and refund eligibility are described in our Refund and Cancellation Policy, which is incorporated into these Terms by reference.

By booking, scheduling, or paying for any Service, you agree to the fees and the Refund and Cancellation Policy posted on PetESALetter.com.

Key Points:

  • Fees are due at the time of booking

  • Cancellations made 24+ hours in advance may be eligible for refund or reschedule

  • Late cancellations and no-shows are generally non-refundable

  • If you do not qualify for an ESA/PSD letter, you may be eligible for a refund under our “No Qualification – Refund Eligible” policy

12. Privacy and Data Protection

Our collection, use, storage, and disclosure of your personal information and Protected Health Information (PHI) are governed by our Privacy Policy, which is incorporated into these Terms by reference.

By using the Services, you acknowledge that you have read and consent to the collection and use of your information as described in the Privacy Policy, in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and other applicable privacy laws.

We do not sell your personal information or PHI to third parties.


13. Intellectual Property

All content on the PetESALetter.com website, including text, graphics, logos, icons, images, and software, is the property of VeriMedix LLC or its licensors and is protected by U.S. and international copyright, trademark, and other intellectual property laws.

You may not copy, reproduce, republish, upload, post, transmit, distribute, modify, create derivative works from, or commercially exploit any content from our website without prior written permission from VeriMedix LLC, except for personal, non-commercial use directly related to your use of the Services.


14. Prohibited Uses

You agree not to:

  • Use the Services or website for any fraudulent, unlawful, misleading, or harmful purpose

  • Impersonate any person or entity, or falsely state or misrepresent your affiliation with any person or entity

  • Attempt to gain unauthorized access to our systems, networks, servers, databases, or other users’ accounts or information

  • Use automated systems (bots, scrapers, etc.) to access the website without our prior written consent

  • Upload, post, or transmit any viruses, malware, or other harmful code

  • Harass, threaten, defame, or otherwise harm our staff, clinicians, or other users

  • Obtain an ESA or PSD letter fraudulently or for any purpose other than legitimate therapeutic and accommodation purposes

Violation of these prohibitions may result in immediate termination of your access and Services, and may be reported to appropriate legal authorities.


15. Disclaimers

TO THE MAXIMUM EXTENT PERMITTED BY APPLICABLE LAW, THE SERVICES AND WEBSITE ARE PROVIDED ON AN “AS IS” AND “AS AVAILABLE” BASIS, WITHOUT ANY WARRANTIES OF ANY KIND, EITHER EXPRESS OR IMPLIED, INCLUDING BUT NOT LIMITED TO WARRANTIES OF MERCHANTABILITY, FITNESS FOR A PARTICULAR PURPOSE, NON-INFRINGEMENT, OR ACCURACY.

VeriMedix LLC and PetESALetter.com do not warrant that:

  • The Services will meet your specific requirements or expectations

  • You will qualify for an ESA letter, PSD documentation, or any specific diagnosis

  • The Services will be uninterrupted, timely, secure, or error-free

  • Any particular outcome, diagnosis, letter, or recommendation will be provided

  • Third parties (landlords, etc.) will accept your documentation


16. Limitation of Liability

TO THE MAXIMUM EXTENT PERMITTED BY LAW, VERIMEDIX LLC, PETESALETTER.COM, AND THEIR RESPECTIVE OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, CONTRACTORS, CLINICIANS, AND AFFILIATES SHALL NOT BE LIABLE FOR ANY INDIRECT, INCIDENTAL, CONSEQUENTIAL, SPECIAL, PUNITIVE, OR EXEMPLARY DAMAGES ARISING OUT OF OR RELATED TO YOUR USE OF THE WEBSITE OR SERVICES, INCLUDING BUT NOT LIMITED TO LOST PROFITS, LOST DATA, PERSONAL INJURY, PROPERTY DAMAGE, OR THIRD-PARTY REJECTION OF DOCUMENTATION, EVEN IF WE HAVE BEEN ADVISED OF THE POSSIBILITY OF SUCH DAMAGES.

OUR TOTAL LIABILITY FOR ANY CLAIM ARISING OUT OF OR RELATING TO THESE TERMS OR THE SERVICES SHALL NOT EXCEED THE AMOUNT YOU PAID TO PETESALETTER.COM FOR THE SPECIFIC SERVICE GIVING RISE TO THAT CLAIM.

Some jurisdictions do not allow the exclusion or limitation of certain damages, so some of the above limitations may not apply to you.


17. Indemnification

You agree to indemnify, defend, and hold harmless VeriMedix LLC, PetESALetter.com, and their respective officers, directors, employees, agents, contractors, clinicians, and affiliates from and against any and all claims, damages, liabilities, costs, and expenses (including reasonable attorneys’ fees and court costs) arising from or related to:

  • Your misuse of the website or Services

  • Your violation of these Terms

  • Your violation of any applicable law or regulation

  • Your misuse, alteration, or fraudulent use of any clinical documentation or letters

  • Any false or misleading information you provide

  • Any harm caused to third parties as a result of your actions


18. Changes to These Terms

VeriMedix LLC reserves the right to update, modify, or replace these Terms at any time, in its sole discretion. The “Last updated” date at the top of this document will indicate the latest revision.

Your continued use of the website or Services after any changes are posted constitutes your acceptance of the revised Terms.

We encourage you to review these Terms periodically.


19. Governing Law and Dispute Resolution

These Terms shall be governed by and construed in accordance with the laws of the State of North Carolina, United States, without regard to its conflict-of-law principles.

Any disputes, claims, or controversies arising out of or relating to these Terms or the Services shall be resolved exclusively in the state or federal courts located in North Carolina, and you consent to the personal jurisdiction of such courts, unless applicable telehealth, consumer protection, or other laws require a different forum.


20. Severability

If any provision of these Terms is found to be invalid, unlawful, or unenforceable by a court of competent jurisdiction, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect.


21. Entire Agreement

These Terms, together with the Refund and Cancellation Policy and Privacy Policy, constitute the entire agreement between you and VeriMedix LLC / PetESALetter.com regarding the use of the website and Services, and supersede any prior agreements or understandings, whether written or oral.
